IF YOU HAVE A SPONSOR PLEASE MAKE SURE IT IS SPELLED CORRECTLY. if your sponsor name is missing please include it on the form.

IF YOU HAVE A SPONSOR BUT THEY HAVE NOT PAID YET PLEASE NOTE THIS ON THE APPROVAL FORM.

If you do not have a sponsor, your team name i.e. Highland Soccer, will be placed where the sponsor name goes.

Once reviewing your poster please fill in the approval form.

Photos cannot be changed. They are the photos chosen by  your child at the shoot. Please do not contact us asking to see different photos. Excess photos may have already been deleted.
